What is Urgency?
Urgency is an abrupt, strong, often overwhelming need to urinate.
Why Do We Experience Urgency?
Urgency can be a symptom of various conditions such as Overactive Bladder (OAB), urinary tract infections (UTIs), prostate or other bladder issues, neurological diseases, diabetes, and abdominal pressure. Certain medications and substances like diuretics, lithium, SSRIs, TCAs, antihistamines, calcium channel blockers, alpha blockers, clonidine, benzodiazepines, caffeine, and alcohol may also contribute to increased urination.
In the case of OAB, oxidative stress is believed to cause bladder muscle stretch receptors to become overly sensitive or damaged. Additionally, nerve problems at both peripheral and central levels can play a role.

What Can Be Done?
- Bladder Training: Keep a diary and set a schedule to retrain the brain and improve control over urination.
- Physical Therapy (PT): Strengthen pelvic floor muscles or relax them if they are tight.
- Medications: Consult with a healthcare provider for appropriate medication options.
- Supplements: Consider magnesium, Vitamin D, and Vitamin C to support bladder health.
- Diet: Opt for a low-carb, natural whole foods diet, eliminate vegetable oils, avoid smoking, and limit caffeine and alcohol intake. Aim for high oxygen and low inflammation.
- Physical Activity: Regular exercise and walking can improve overall health and reduce symptoms.

Treating Urinary Urgency Incontinence with the Axonics Rechargeable Sacral Neuromodulation System
The ARTISAN-SNM study evaluated the effectiveness of the Axonics rechargeable sacral neuromodulation system in treating urinary urgency incontinence (UUI) over a two-year period. The study included 129 participants.
Key Findings:
- Responder Rate: At the two-year mark, 93% (121 out of 129) of participants were responders.
- Reduction in UUI: 93% of participants experienced more than a 50% reduction in UUI episodes.
- UUI Episodes: There was a 75% reduction in UUI episodes across all study participants, with daily episodes decreasing from 5.6 at baseline to 1.0 at two years.
- Frequency of Urination: Initially, 80% of participants had more than 8 urinary incontinence episodes per day, with an average of 11.6 episodes per day. This was reduced to an average of 8.6 episodes per day at two years.
- Large Leaks: The number of large leaks significantly decreased from 1.6 at baseline to 0.1 at two years for the 76 participants who had large leaks initially.
- Patient Satisfaction: 94% of participants reported being satisfied with the treatment.
